What’s inside
Most “how much does Google Ads management cost” articles are written for local plumbers and SaaS startups. If you sell peptides, supplements, CBD or anything else Google treats as healthcare-adjacent, the economics are different: the work starts before the first click, mistakes cost you the account rather than a bad month, and the cheapest option is often the most expensive one. This guide explains the models honestly and shows exactly what we charge.
The four pricing models, compared
| Model | How it works | Good for | Watch out for |
|---|---|---|---|
| Percentage of spend | Fee = a % of monthly media spend, often 10–20%, frequently with a minimum | Accounts expected to scale; aligns effort with account size | Incentive to raise spend; flat % gets expensive at high budgets unless it tiers down |
| Flat monthly fee | Fixed retainer regardless of spend | Stable budgets; predictable costs | Can become under-resourced as you scale; check what’s capped |
| Hybrid | Base retainer plus a smaller % above a threshold | Growing accounts wanting predictability | Make sure the base covers real work, not just reporting |
| Performance-based | Fee tied to revenue, ROAS or leads | Mature accounts with clean attribution | Disputes over attribution; agencies may avoid long-term compliance work that doesn’t pay back this month |
None of these is “right.” A sliding percentage, where the rate falls as spend rises, is the model we chose because it keeps small accounts viable for us to service properly while not penalising brands as they scale.
Why regulated brands pay more (and should)
In a typical e-commerce account, a new agency can be running campaigns within a week. In a peptide or supplement account, the first job is making sure the business can advertise at all without being suspended. That involves:
- Policy review against Google’s Healthcare and medicines rules, which prohibit unapproved substances “regardless of any claims of legality.”
- Website and landing-page fixes so the site, feed and Merchant Center tell one consistent story. Inconsistency causes misrepresentation suspensions.
- Feed engineering: titles, attributes and product data written to Google’s standards. See Merchant Center for research peptides.
- Certification checks where they apply, such as Healthcare and medicines or LegitScript routes.
- Ongoing monitoring, because Google’s enforcement changes and a single disapproved asset can cascade.
A suspension switches off Shopping ads and free listings at once. For a brand doing $50,000 a month through Google, a two-week outage costs more than a year of the difference between a cheap and a specialist agency, before counting the risk of a permanent ban. See why cheap fixes backfire.
Our pricing, with worked examples

Setup: from $2,000 one-time. Covers compliance audit and rebuild, Merchant Center feed engineering, campaign architecture and conversion tracking.
| Monthly ad spend | Management fee |
|---|---|
| Under $5,000 | 20% (minimum $1,000/month) |
| $5,000 – $15,000 | 15% |
| $15,000 – $30,000 | 10% |
| $30,000 – $75,000 | 7.5% |
| $75,000+ | 5% |
| Example spend | Rate | Monthly fee | Fee as % of spend |
|---|---|---|---|
| $3,000 | 20% (minimum applies) | $1,000 | 33% |
| $10,000 | 15% | $1,500 | 15% |
| $25,000 | 10% | $2,500 | 10% |
| $50,000 | 7.5% | $3,750 | 7.5% |
| $100,000 | 5% | $5,000 | 5% |
The fee is calculated on actual spend each month. There are no long-term contracts. Your exact quote is confirmed on a 15-minute call, because catalogue size and compliance state affect setup.
At $3,000/month, a $1,000 fee is a third of your media budget. If you’re at that level, the honest advice is often to fix the site and feed first, run a tightly scoped brand and high-intent campaign, and scale once it’s profitable. We’ll tell you on the call if we don’t think you’re ready.
What the fee should include
Whatever an agency charges, these are the things a regulated brand should expect inside the management fee, not as extras:
- Merchant Center and feed management, including disapproval handling.
- Policy monitoring and immediate response to disapprovals or warnings.
- Suspension diagnosis and appeal for managed clients. Ours is included; standalone it’s $1,500.
- Conversion tracking that is accurate and privacy-compliant.
- Search, Shopping and, where appropriate, Performance Max structure. See our campaign structure by catalogue size.
- Weekly optimisation and a monthly review with a senior person, not just a report.
- Access: you own the Google Ads and Merchant Center accounts, always.
Questions to ask any agency
- Who owns the ad account and Merchant Center if we part ways?
- Have you worked in my category, and which products do you think Google won’t accept?
- Is setup separate? What exactly does it deliver?
- Does the percentage tier down as spend grows?
- What happens, and what does it cost, if we get suspended?
- Who actually works on the account day to day?
- What’s the notice period?
15 questions answered
How much does Google Ads management cost per month?
Commonly 10–20% of ad spend, or a flat retainer, often with a minimum. For regulated categories, add a one-time setup for compliance work. At iComChain the management fee runs from 20% under $5,000/month (minimum $1,000) down to 5% above $75,000/month.
Is percentage of spend or a flat fee better?
Neither is universally better. A tiered percentage scales with account effort and gets cheaper as you grow; a flat fee is predictable. Check what each includes and whether the percentage tiers down.
Why is there a setup fee?
Regulated accounts need policy review, site and landing-page fixes, feed engineering and tracking before spending is safe. That’s front-loaded work, separate from ongoing management.
What does the $2,000 setup include?
A compliance audit and rebuild, Merchant Center feed engineering, campaign build and conversion tracking. Larger catalogues or heavier site work can increase it, which is why it’s quoted as $2,000+.
Is ad spend included in the management fee?
No. Ad spend is paid directly to Google from your account. The management fee is separate and calculated on actual spend.
What is the minimum monthly fee?
$1,000 per month, which applies when 20% of your spend would be less than that, i.e. below $5,000/month in spend.
Do you require a long-term contract?
No. Engagements are month to month.
Is Merchant Center suspension recovery included?
For managed clients, yes: diagnosis, appeals and weekly policy scans are part of management. Standalone reinstatement is a flat $1,500.
Can peptide brands actually run Google Ads?
Some can, within strict limits. Many peptides are effectively unadvertisable on Google regardless of copy. We assess which products and campaign types are viable before you spend anything. See Google Ads policy for peptides.
How fast will I see results?
Brand and high-intent search can show data within the first weeks. Shopping depends on feed approval and the new-account ramp, which can take several weeks. See new-account limits.
Do I own the ad account?
Yes. We work in accounts you own, and you keep full access if we part ways.
Do you charge on top of Performance Max or Shopping separately?
No. The management percentage covers all Google Ads campaign types we run for you.
Do you offer website builds too?
Yes. Compliant website builds with an AI automation layer start at $4,000. Custom B2B AI agents start at $5,000.
Why not choose a performance-only agency?
Performance-only models can work for mature accounts, but in regulated categories much of the value is protecting the account, which doesn’t show up as this month’s revenue. Make sure compliance work isn’t being skipped.
